We register new patients who live within our practice area.  To help us give you the best service possible we would encourage you to do this as soon as possible.

When you register with the practice you will be asked to complete a registration form known as a GMS1 and a medical questionnaire. This is because it can take a considerable time for us to receive all your medical records from your previous surgery. Medical treatment is available from the date of registration.

To register with the surgery, you can either:

  •     Complete the forms at reception
  •     Download the forms below and return to reception - hnyicb-voy.b82097reception@nhs.net

It would help us when processing your application if you included some photo ID and proof of address, however this is not a legal requirement, and your registration will not be delayed if these aren't provided.

GP to GP Record Transfers

NHS England requires practices to utilise the GP2GP facility for the transfer of patient records between practices when a patient registers.

 

It is very important that you are registered with a doctor at all times. If you leave your previous practice and register with a new GP, your medical records will be removed from your previous doctor and forwarded on to your new GP via NHS England. It can take several months for your paper records to reach your new surgery. However, GP2GP record transfers allow your electronic medical record to be transferred to your new practice much sooner. 

GP2GP transfers are already active at Scott Road Medical Centre, and we send and receive patient records via this system.
