New Patient Registrationregister_now

We register new patients who live in Selby or in a village within a 7 mile radius of the practice.

To help us give you the best service possible we would encourage you to do this as soon as possible – don’t wait until the first winter bug hits! You don’t have to be ill to register. However, if you are ill and you ring up for an appointment please inform the receptionist that you want to register as more time will be needed. 

When you register with the practice you will be asked to complete a registration form known as a GMS1.

You will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records.

You can either call in at reception to complete these forms which may take some time or alternatively click on the link below to print the form out in order to complete before returning them to us.

Please return the completed forms to reception or email to them voyccg.b82097reception@nhs.net 

It would help us when processing your application if you included some photo ID and proof of residency, however this is not a legal requirement and your registration will not be delayed if these aren't provided.
